Service Description

Visit Prep: 1. Must present a valid driver's license at time of examination, so a copy can be made for our records (Visit Prep: 1. Must present a valid driver's license at time of examination, so a copy can be made for our records (as required by the FMCSA). 2. Avoid eating or drinking liquids that contain sugar. This will decrease the risk of sugar being detected when your urine is tested. 3. Limit your caffeine and salty food intake to help keep your blood pressure at healthy levels. 4. If you smoke, work towards quitting to see healthier blood pressure levels. 5. Walking as a form of exercise will help manage your weight while decreasing blood pressure and blood sugar levels. 6. If you are currently taking any prescription medications, be sure to continue taking them as prescribed. 7. Bring a list of all medications and their dosage with you for your physical exam. 8. If you are diabetic, you must bring a lab sheet with your hemoglobin A1c results from within 3 months of your physical exam date. NOTE—If you do not have current hemoglobin A1c results and are unable to obtain them prior to your physical exam, our office can perform the testing for an additional fee. 9. If you are taking Coumadin / Warafrin, you must bring a lab sheet with your INR results from within 2 weeks of your physical exam date. 10. Be sure to bring your prescription lenses (glasses or contacts) if they are required for driving. If you require reading glasses, please bring them to assist with completing your paperwork. 11. If you require hearing devices, you will need them to complete your whisper hearing test. 12. If you require sleep therapy to treat sleep apnea, you must provide a clearance letter from your treating medical provider or data from your CPAP machine. NOTE—Drivers can bring their Philips or ResMed CPAP machine SD card to their visit for our office to download and review the data. If your machine uploads wirelessly, we will be able to access the data once you provide the requested information. 13. If you have any current medial conditions, the FMCSA requires a medical clearance form be filled out by your treating medical provider, in order to complete your physical exam. NOTE—Medical clearance forms are available on our website (under the "DOT Forms" tab) for you to print or email to your treating medical provider. The completed form can be sent directly to us by the treating medical provider, or returned by you on your physical exam date.
