Visit Prep: 1. Must present a valid photo ID at time of draw. 2. Must present a Quest Diagnostics “Quest Select” Lab Card at time of draw. 3. Must present a lab work order sheet at time of draw. 4. Be sure to fast if the issuing provider or order sheet indicate to do so. If your order sheet or verbal instructions were not clear, please contact our office for further assistance.
