Service Description

Visit Prep: 1. Must present a valid photo ID of the patient or parental / legal guardian at time of visit. 2. Be prepared to provide a detailed medical history. 3. Be prepared to provide a detailed surgical history. 4. Be prepared to provide a detailed list of medications (preferably with dosage). 5. Be prepared to list all known allergies. 6. We are a self-pay clinic. Cash, credit card, FSA/HSA cards, or contactless payment are the only accepted forms of payment. 7. If you are being seen for a work-related injury (workers comp) our office will need to receive prior authorization from your employer before you can be evaluated and receive care.
